- The Doctor General Practitioner shall maintain a valid and active license issued by the Saudi Medical Commission throughout the duration of this Purchase Order. Any lapse in licensure shall constitute grounds for immediate termination.
- All services shall be rendered at the designated clinic premises located in Jeddah, Saudi Arabia, unless otherwise agreed in writing by both parties.
- The Doctor General Practitioner shall comply with all data protection and patient confidentiality requirements as stipulated by the Saudi Personal Data Protection Law (PDPL) and the Ministry of Health guidelines.
- Payment shall be processed via bank transfer to the account designated by the Doctor General Practitioner within thirty (30) calendar days of receipt of a valid invoice referencing this Purchase Order number (PO-JED-2025-04782).
- This Purchase Order is subject to the laws of the Kingdom of Saudi Arabia. Any disputes arising from this Purchase Order shall be resolved through the competent courts in Jeddah, Saudi Arabia, or through arbitration as per the Saudi Arbitration Center rules.
- The Doctor General Practitioner shall submit monthly service reports detailing patient volume, referral outcomes, and any incidents or adverse events to the procurement department of Al-Shifa Medical Group, Jeddah Branch.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, all prepaid services for the remaining period shall be refunded pro-rata.
- All equipment and consumables procured under this Purchase Order shall be delivered to the clinic address in Jeddah within fourteen (14) business days of the Purchase Order acceptance date.
All physical goods listed in this Purchase Order shall be delivered to the Al-Shifa Medical Group facility at King Fahd Road, Al-Salam District, Jeddah 23412, Saudi Arabia. The Doctor General Practitioner and the designated clinic manager shall inspect all delivered items within five (5) business days of delivery. Any discrepancies, damages, or non-conformities must be reported in writing to the procurement department within the same period. Failure to report within this window shall constitute acceptance of the goods as delivered.
This Purchase Order is authorized and executed by the undersigned representatives of both parties. By signing below, each party acknowledges and agrees to all terms, conditions, and obligations set forth in this Purchase Order for the engagement of the Doctor General Practitioner and the procurement of associated services and goods in Saudi Arabia, Jeddah.
