All Doctor General Practitioner services referenced in this Purchase Order shall be delivered at the Singapore Singapore Medical Centre premises or at the Doctor General Practitioner clinic located within the Singapore Singapore district, as mutually agreed upon. The initial Doctor General Practitioner health screening sessions shall commence no later than 20 June 2025. The chronic disease management program under this Purchase Order shall be rolled out in phases over a twelve-month period. All medical equipment and consumables listed in Item 05 must be delivered and installed at the Singapore Singapore Medical Centre Doctor General Practitioner consultation rooms by 25 June 2025.
6.1 This Purchase Order is governed by the laws of the Republic of Singapore. Any disputes arising from this Purchase Order relating to Doctor General Practitioner services shall be resolved through arbitration in Singapore Singapore in accordance with the rules of the Singapore International Arbitration Centre (SIAC).
6.2 The Doctor General Practitioner service provider shall maintain valid medical registration with the Singapore Medical Council and shall ensure that all Doctor General Practitioner consultations are conducted by licensed practitioners. Proof of registration shall be provided upon request by Singapore Singapore Medical Centre.
6.3 Payment shall be made within thirty (30) days of receipt of a valid tax invoice from the Doctor General Practitioner service provider. Late payments shall attract interest at the rate of 5% per annum as stipulated under the Singapore Interest Act.
6.4 The Doctor General Practitioner service provider warrants that all services and supplies under this Purchase Order shall meet the quality standards set by the Health Sciences Authority (HSA) of Singapore Singapore. Non-conforming items shall be replaced or refunded within fourteen (14) days of written notice.
6.5 This Purchase Order may be amended only by mutual written agreement between Singapore Singapore Medical Centre and the Doctor General Practitioner service provider. No verbal modifications shall be binding.
6.6 The Doctor General Practitioner service provider shall comply with all data protection requirements under the Personal Data Protection Act (PDPA) of Singapore Singapore when handling patient records generated under this Purchase Order.
By signing below, both parties acknowledge and accept the terms, conditions, and scope of work outlined in this Purchase Order. The Doctor General Practitioner service provider confirms capacity to deliver all services within the Singapore Singapore jurisdiction as specified. Singapore Singapore Medical Centre confirms that the funds for this Purchase Order have been allocated in the fiscal year 2025 budget.
