Note: All items listed in this Purchase Order are intended for the operational use of a registered Doctor General Practitioner practice located in Switzerland Zurich. The recipient, Dr. Elisabeth Müller, holds a valid FMH (Föderation des Médecins Suisses) license for general practice. All medical devices must comply with Swissmedic regulations and carry the CE marking as required for distribution within Switzerland.- 1. Delivery: All goods specified in this Purchase Order shall be delivered to the Doctor General Practitioner practice at Seefeldstrasse 87, 8008 Zurich, Switzerland. Delivery must be completed no later than 28 June 2025. The supplier is responsible for all transportation costs within the Zurich metropolitan area.
- 2. Quality Assurance: All medical supplies and equipment must be new, unused, and in full compliance with Swiss medical device regulations (MedDevV). The Doctor General Practitioner reserves the right to inspect all items upon delivery and reject any goods that do not meet the specified standards or Swissmedic requirements.
- 3. Payment: Payment of CHF 6,275.38 shall be made via bank transfer to the supplier's account within 30 days of the invoice date. The bank details are: MedSupply AG, UBS AG, IBAN: CH93 0023 0456 7800 3890 7, BIC: UBSWCHZH80A, Zurich, Switzerland.
- 4. Cancellation & Modification: This Purchase Order may be modified or cancelled in writing by either party up to 5 business days prior to the required delivery date. Any cancellation after this period may be subject to a 15% restocking fee on non-returnable items.
- 5. Warranty: All electronic medical devices (items 01, 02, 04, 05) carry a minimum 24-month manufacturer warranty. The Doctor General Practitioner shall notify MedSupply AG of any defects within 14 days of discovery.
- 6. Governing Law: This Purchase Order is governed by the laws of Switzerland. Any disputes arising from this order shall be resolved by the competent courts in Zurich, Switzerland. The parties agree to attempt mediation before initiating legal proceedings.
- 7. Data Protection: In accordance with the Swiss Federal Act on Data Protection (FADP/DSP), all personal data exchanged in connection with this Purchase Order shall be processed in compliance with applicable Swiss data protection legislation.
- 8. Regulatory Compliance: The supplier warrants that all items are legally importable and distributable within Switzerland and meet the requirements for use by a Doctor General Practitioner in clinical practice in the Canton of Zurich.
