Terms and Conditions
- This Purchase Order constitutes a formal engagement of the named Doctor General Practitioner to provide clinical and community health services within the Dar es Salaam region of Tanzania, in accordance with the Tanzania Medical and Dental Practitioners Act (Cap. 361) and regulations issued by the Medical Council of Tanzania.
- The Doctor General Practitioner shall maintain a valid and current registration with the Medical Council of Tanzania throughout the duration of this Purchase Order. Any lapse in registration shall constitute grounds for immediate termination without penalty to the issuing organization.
- All clinical services rendered by the Doctor General Practitioner shall be conducted in compliance with the Tanzania Health Policy 2022–2030 and the Dar es Salaam Regional Health Strategic Plan. Patient records shall be maintained in accordance with the Tanzania Data Protection Act, 2022.
- Payment shall be processed within thirty (30) calendar days of receipt of a valid invoice and supporting documentation, including the Doctor General Practitioner's monthly service report and any relevant patient outcome summaries. Payments shall be made via bank transfer to the account designated by the Doctor General Practitioner in Dar es Salaam.
- The Doctor General Practitioner is required to submit a comprehensive monthly report detailing patient consultations, diagnoses, referrals, community outreach activities, and any adverse events to the Medical Director of Mwalimu Health & Wellness Centre, Dar es Salaam, no later than the fifth (5th) business day of the following month.
- All medical equipment and consumables procured under this Purchase Order shall be delivered to the practice location at 22 Mbezi Beach Road, Masaki, Dar es Salaam, Tanzania. The Doctor General Practitioner shall confirm receipt and condition of all items within five (5) business days of delivery.
- This Purchase Order is subject to the standard terms of the Tanzania Public Procurement Act, 2011 (as applicable to private sector procurement) and the Civil and Commercial Code of Tanzania. Any disputes arising from this Purchase Order shall be resolved through arbitration in Dar es Salaam, Tanzania, in accordance with the Arbitration Act, Cap. 20.
- The Doctor General Practitioner shall not subcontract or delegate any clinical duties specified in this Purchase Order without prior written consent from the issuing organization. All services must be personally rendered by the registered Doctor General Practitioner named herein.
- This Purchase Order may be terminated by either party with thirty (30) days' written notice. In the event of termination, all outstanding payments for services rendered up to the termination date shall be settled in full within fourteen (14) days.
- All communications regarding this Purchase Order shall be directed to the procurement office of Mwalimu Health & Wellness Centre, Dar es Salaam, Tanzania, and shall be in the English language, which is the official language of business in Tanzania.
