- This Purchase Order is issued by Lakeside Hair & Beauty Studio, a licensed hairdressing establishment operating in Chicago, Illinois, United States, and is governed by the laws of the State of Illinois and applicable federal regulations.
- All items listed in this Purchase Order are intended for professional use by licensed hairdressers and cosmetologists at the buyer's Chicago, United States location. The vendor warrants that all products comply with the Illinois Department of Public Health regulations and the Chicago Department of Public Health sanitation and safety codes.
