The Laboratory Technician engaged under this Purchase Order shall perform the following duties at the Buyer's facility in United States San Francisco:
- Collect, process, and analyze clinical specimens including blood, urine, tissue, and microbiological samples in compliance with CLIA '88 standards.
- Operate, calibrate, and maintain automated and manual laboratory instrumentation, ensuring all equipment meets the quality assurance requirements of the California Department of Public Health.
- Perform quality control procedures daily and document all results in the laboratory information system (LIS).
- Adhere to all biosafety protocols and OSHA regulations applicable to laboratory environments in San Francisco, California.
- Collaborate with pathologists, clinical chemists, and other laboratory personnel to ensure accurate and timely diagnostic reporting.
- Maintain a clean, organized, and compliant laboratory workspace in accordance with the City and County of San Francisco health and safety codes.
- Payment Terms: Net 30 days from the date of invoice. All payments shall be made via ACH transfer to the Vendor's designated account. Invoices must reference this Purchase Order number (PO-2025-SF-04872) for processing.
- Commencement Date: The Laboratory Technician shall begin active service no later than July 1, 2025, at the San Francisco laboratory facility. A two-week orientation period shall precede full clinical duties.
- Contract Duration: This Purchase Order covers a twelve (12) month term commencing July 1, 2025, and expiring June 30, 2026. Renewal shall be subject to mutual written agreement and a new Purchase Order issuance.
- Compliance: The Vendor warrants that the Laboratory Technician holds a current California Clinical Laboratory Technologist (CLT) or Medical Laboratory Technician (MLT) license issued by the California Department of Industrial Relations, Division of Labor Standards Enforcement. All personnel shall comply with the San Francisco Fair Chance Ordinance and applicable federal employment laws.
- Insurance: The Vendor shall maintain general liability insurance of no less than $2,000,000 per occurrence and workers' compensation insurance as required by California Labor Code, covering the Laboratory Technician for the duration of this engagement in United States San Francisco.
- Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the Buyer shall pay for all services rendered through the termination date, including a pro-rated portion of the administrative fee.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the State of California. Any disputes shall be resolved in the Superior Court of California, County of San Francisco.
