- This Purchase Order constitutes a binding agreement between Capital Health & Maternity Services (Pvt.) Ltd. and the named Midwife provider for the duration specified herein. All services shall be rendered within the jurisdiction of Pakistan Islamabad unless otherwise approved in writing.
- The Midwife shall maintain a valid PMDC registration and professional liability insurance throughout the term of this Purchase Order. Any lapse in licensure shall result in immediate suspension of services.
- Payment shall be made in monthly installments within 30 days of receipt of a valid tax invoice. The total value of this Purchase Order is PKR 2,230,200 (Two Million Two Hundred Thirty Thousand Two Hundred Pakistani Rupees) inclusive of all applicable taxes.
- The Midwife shall comply with all data protection regulations of the Government of Pakistan Islamabad, including the Personal Data Protection Bill, and shall maintain strict confidentiality of all patient records.
- Service delivery shall adhere to the National Maternal Health Strategy of Pakistan Islamabad and the clinical protocols established by the facility. The Midwife shall submit weekly activity reports to the Maternal & Child Health Division.
- Either party may terminate this Purchase Order with 30 days written notice. In the event of termination, payment shall be made pro-rata for services rendered up to the termination date.
- All disputes arising under this Purchase Order shall be resolved through arbitration in Pakistan Islamabad in accordance with the Arbitration Act, 1940 of Pakistan.
- The Midwife shall not subcontract any portion of the services outlined in this Purchase Order without prior written consent from the issuing authority.
All services under this Purchase Order shall be primarily delivered at the Capital Maternity Wing, Plot 47-B, Blue Area, Pakistan Islamabad, and at designated community health centers within the Islamabad Capital Territory. Home visits by the Midwife shall be conducted within a 25-kilometer radius of the main facility in Pakistan Islamabad.
