- Scope of Midwife Services: This Purchase Order authorizes the engagement of licensed Midwife professionals through Midwife Care Solutions of Miami, Inc. to provide comprehensive obstetric and perinatal care at the purchasing party's facility located in United States Miami. All Midwife practitioners must hold active certification from the American College of Nurse-Midwives (ACNM) and maintain a valid Florida Board of Nursing license.
- Compliance: All Midwife services rendered under this Purchase Order shall comply with the Florida Statutes Chapter 456, the Florida Administrative Code, and all applicable regulations of the United States Miami-Dade County Health Department. The Midwife provider shall maintain professional liability insurance with a minimum coverage of $2,000,000 per occurrence.
