• 4.2 Service Period: The Midwife engagement under this Purchase Order is valid from 1 July 2025 through 30 June 2026. Any extension beyond this period requires a written amendment to this Purchase Order signed by both parties.
• 4.3 Cancellation & Termination: Either party may terminate this Purchase Order with a written notice of thirty (30) days. In the event of early termination, the Midwife shall be compensated for all services rendered up to the termination date, and no further Midwife service fees shall be due.
• 4.4 Confidentiality: The Midwife shall maintain strict confidentiality regarding all patient information in compliance with Vietnam's Personal Data Protection Law. Breach of confidentiality shall result in immediate termination of this Purchase Order and potential legal action.
• 4.5 Quality Assurance: The Midwife shall undergo quarterly performance evaluations. Failure to meet the minimum service standards outlined in the attached Service Level Agreement (SLA) may result in a formal notice and, if unresolved within fifteen (15) days, termination of this Purchase Order.
