This Purchase Order is issued by the Harare Provincial Maternal Health Authority to procure the professional services of a qualified Midwife to be deployed at the Harare Central Maternity Clinic, located in Zimbabwe Harare. The Midwife shall provide comprehensive antenatal, intrapartum, and postnatal care in accordance with the standards set by the Zimbabwe National Health Research Council and the Ministry of Health and Child Care, Zimbabwe. The Midwife must hold a valid registration with the Zimbabwe Nursing Council and possess a minimum of five (5) years of clinical experience in obstetric and neonatal care within the Zimbabwe Harare metropolitan area.

- This Purchase Order constitutes a binding agreement between the Harare Provincial Maternal Health Authority and Harare Midwifery & Maternal Care Services (Pvt) Ltd for the provision of Midwife services in Zimbabwe Harare.
- The Midwife engaged under this Purchase Order shall be fully registered with the Zimbabwe Nursing Council and shall maintain valid professional indemnity insurance throughout the contract period.
- Payment shall be made in United States Dollars (USD) on a monthly basis within fourteen (14) calendar days of receipt of a valid invoice from the service provider. All payments shall be processed through the Central Bank of Zimbabwe (CBZ) Harare Branch.
- The Midwife shall report to the Harare Central Maternity Clinic, 45 Avondale Road, Harare, Zimbabwe, on a full-time basis (Monday to Saturday, 07:00 to 19:00) and shall be available for emergency call-outs as required by the clinic management.
- The service provider shall ensure that the Midwife complies with all applicable laws and regulations of the Republic of Zimbabwe, including the Health Professions Act, the Labour Act, and the National Health Services Act.
- Any changes to the scope of Midwife services outlined in this Purchase Order must be approved in writing by both parties prior to implementation. No verbal amendments shall be valid.
- The Purchase Order may be terminated by either party with thirty (30) days written notice. In the event of termination, the service provider shall be compensated for services rendered up to the termination date.
- All patient records and clinical data generated by the Midwife during the course of this engagement shall remain the property of the Harare Provincial Maternal Health Authority and shall be stored in accordance with Zimbabwe data protection regulations.
- The service provider shall submit a comprehensive monthly report detailing the number of antenatal visits, deliveries, postnatal checks, and community outreach sessions conducted by the Midwife in Zimbabwe Harare.
- Disputes arising from this Purchase Order shall be resolved through mediation in Harare, Zimbabwe, in accordance with the Zimbabwe Mediation Act. If mediation fails, the matter shall be referred to the High Court of Zimbabwe, Harare.
- This Purchase Order is governed by the laws of the Republic of Zimbabwe.
