Terms and Conditions
- This Purchase Order is issued by St. Anselm Medical Center, Inc., located in Philippines Manila, and governs the procurement of qualified Nurse personnel and associated services for a period of twelve (12) months commencing July 1, 2025.
- All Nurse candidates must present a valid and unexpired Professional Regulation Commission (PRC) License, a Certificate of Good Moral Character from the Philippine National Police, and a recent medical examination certificate from a licensed physician in the Philippines prior to the start date.
- The vendor, CareBridge Healthcare Staffing Solutions, Inc., shall bear full responsibility for the recruitment, screening, and initial placement of all Nurse personnel. Any replacement of a Nurse due to performance or conduct issues within the first 90 days shall be provided at no additional cost to the buyer.
- Payment shall be made via bank transfer to the vendor's designated account within thirty (30) calendar days from the receipt of a valid, itemized invoice. All invoices must reference this Purchase Order number (SAMC-2025-00487) and comply with the Bureau of Internal Revenue (BIR) requirements for the Philippines Manila jurisdiction.
- The vendor shall comply with all applicable labor laws of the Republic of the Philippines, including the Labor Code, the Philippine Nursing Act of 2002 (Republic Act No. 9173), and all local ordinances of the City of Manila and Metro Manila.
- Confidentiality: All patient information and institutional data encountered by the Nurse personnel during the term of this Purchase Order shall be treated as strictly confidential in accordance with the Philippine Data Privacy Act of 2012 (Republic Act No. 10173).
- This Purchase Order may be terminated by either party with a written notice of thirty (30) days. In the event of termination, the vendor shall ensure a smooth transition of all Nurse duties and patient care responsibilities at the Philippines Manila facility.
- Any disputes arising from this Purchase Order shall be resolved through amicable negotiation. Should negotiation fail, the matter shall be submitted to the courts of competent jurisdiction in Philippines Manila.
- This Purchase Order constitutes the entire agreement between the parties regarding the procurement of Nurse services and supersedes all prior negotiations, representations, or agreements, whether written or oral.
