This Purchase Order is issued to procure the professional services of a qualified Occupational Therapist to be deployed and stationed in Afghanistan Kabul for a period of twelve (12) consecutive months. The Occupational Therapist shall provide comprehensive rehabilitation, functional assessment, adaptive equipment training, and therapeutic intervention programs to patients with physical disabilities, neurological impairments, and post-traumatic injuries at the ARHDA-affiliated rehabilitation center located in the Wazir Akbar Khan District of Afghanistan Kabul.

The Occupational Therapist assigned under this Purchase Order shall perform the following duties within the operational boundaries of Afghanistan Kabul:
- Conduct individualized occupational therapy assessments for a minimum of forty (40) patients per month at the designated rehabilitation facility in Afghanistan Kabul.
- Develop and implement personalized treatment plans focusing on restoring functional independence in daily living activities, work-related tasks, and community participation.
- Provide training and capacity-building sessions to local healthcare staff in Afghanistan Kabul on evidence-based occupational therapy techniques and adaptive strategies.
- Coordinate with multidisciplinary teams including physiotherapists, speech-language pathologists, and social workers operating within the Afghanistan Kabul healthcare network.
- Submit monthly progress reports, patient outcome data, and program evaluation documents to the ARHDA procurement office in Afghanistan Kabul by the fifth business day of each subsequent month.
- Ensure all clinical practices comply with the Ministry of Public Health regulations of the Islamic Emirate of Afghanistan and international occupational therapy standards as set forth by the World Federation of Occupational Therapists (WFOT).
- Payment Terms: Payment shall be made in quarterly installments within thirty (30) days of receipt of an approved invoice. The first installment of USD 83,100.00 shall be due upon confirmed commencement of the Occupational Therapist services in Afghanistan Kabul.
- Commencement Date: The Occupational Therapist services shall commence no later than 1 August 2025 in Afghanistan Kabul. Any delay beyond this date shall be subject to a penalty of 0.5% of the total Purchase Order value per day of delay.
- Termination: Either party may terminate this Purchase Order with sixty (60) days written notice. In the event of force majeure affecting operations in Afghanistan Kabul, either party may suspend obligations without penalty.
- Insurance and Liability: The supplier shall maintain comprehensive professional liability insurance covering the Occupational Therapist personnel for the full duration of service delivery in Afghanistan Kabul, with a minimum coverage of USD 2,000,000 per occurrence.
