This Purchase Order authorises the procurement of professional occupational therapy services to be delivered by a qualified Occupational Therapist operating within the Australia Brisbane metropolitan area. The Occupational Therapist shall hold current registration with the Australian Health Practitioner Regulation Agency (AHPRA) under the National Registration and Accreditation Scheme, maintain professional indemnity insurance of no less than $10,000,000, and comply with all applicable Queensland Health and Human Services guidelines. All clinical services must be delivered in accordance with the Australian Occupational Therapy Association (AOTA) Code of Ethics and the National Occupational Therapy Practice Framework. The service delivery area encompasses all suburbs within the Greater Brisbane region, with a primary focus on the inner-city and river-precinct communities of Australia Brisbane.
- This Purchase Order is governed by the laws of the State of Queensland, Australia, and the Commonwealth of Australia. Any disputes arising from this Purchase Order shall be resolved in accordance with the Australian Consumer Law (2010) and the Queensland Civil and Administrative Tribunal (QCAT) procedures.
- The Occupational Therapist engaged under this Purchase Order must maintain a minimum of 40 clinical hours per week and provide a written weekly activity report to the purchasing entity's Clinical Services Director.
- All services must be delivered within the Australia Brisbane metropolitan area unless prior written approval is obtained from the purchasing entity for travel to regional Queensland locations.
- Payment shall be made via direct bank transfer within 30 calendar days of receipt of a valid tax invoice. Late payments shall accrue interest at the rate prescribed under the Queensland Interest on Late Payments of Government Debts Act 1983.
- The service provider warrants that the Occupational Therapist assigned to this Purchase Order shall not be concurrently engaged by a competing allied health provider within the Australia Brisbane region without prior written consent.
- Either party may terminate this Purchase Order with 60 days' written notice. In the event of termination, all outstanding invoices for services rendered up to the termination date shall be settled within 30 days.
- The service provider shall comply with all Australian Privacy Act 1988 requirements and Queensland Health privacy policies when handling client records and clinical data generated during the delivery of occupational therapy services.
- This Purchase Order does not constitute an employment relationship. The Occupational Therapist is engaged as an independent contractor or through the service provider entity as specified herein.
- All intellectual property, clinical protocols, and programme materials developed specifically for the purchasing entity under this Purchase Order shall remain the property of the purchasing entity.
- The service provider shall provide a comprehensive induction and orientation plan for the Occupational Therapist within the first two weeks of commencement, ensuring familiarity with the Australia Brisbane service network, referral pathways, and local community resources.
By signing below, both parties acknowledge and accept the terms, conditions, and scope of work outlined in this Purchase Order for the engagement of Occupational Therapist services in Australia Brisbane. This document constitutes a binding agreement upon execution by both authorised representatives.
