- This Purchase Order is issued in accordance with the Victorian Government Procurement Act 2003 and applicable Australian federal procurement guidelines. All services described herein shall be delivered within the Australia Melbourne metropolitan area unless otherwise agreed in writing.
- The Occupational Therapist named in this Purchase Order must maintain current AHPRA registration, professional indemnity insurance (minimum AUD 10,000,000), and public liability insurance (minimum AUD 20,000,000) for the entire duration of the engagement in Australia Melbourne.
- Payment shall be made within thirty (30) calendar days of receipt of a valid tax invoice referencing this Purchase Order number (PO-2025-MEL-OT-0472). Invoices must be submitted to the Melbourne Allied Health Services Group accounts payable department at the Australia Melbourne address listed above.
- The Occupational Therapist shall comply with all clinical governance standards set by the Australian Health Practitioner Regulation Agency (AHPRA), the Occupational Therapy Australia (OTA) Code of Ethics, and the Victorian Health Services Act 1988.
- All client records, assessment data, and intervention documentation generated under this Purchase Order shall be stored in accordance with the Privacy Act 1988 (Cth) and the Victorian Health Records Act 2009. Data shall not be transferred outside Australia Melbourne or Australia without explicit written consent from the client and the Buyer.
- The Buyer reserves the right to audit the Occupational Therapist's service delivery, clinical notes, and time records at reasonable notice. The Occupational Therapist shall maintain all records for a minimum of seven (7) years in accordance with Australian health record retention standards.
- Any variation to the scope, quantity, or pricing outlined in this Purchase Order must be documented in a written variation order signed by both parties before additional services are rendered.
- This Purchase Order is governed by the laws of the State of Victoria, Australia Melbourne, and the Commonwealth of Australia. Any disputes shall be resolved through the Victorian Civil and Administrative Tribunal (VCAT) or the Federal Court of Australia as appropriate.
- The Occupational Therapist warrants that all services will be performed with the skill, care, and diligence expected of a registered Occupational Therapist practising in Australia Melbourne, and in accordance with the latest evidence-based clinical practice guidelines published by Occupational Therapy Australia.
- This Purchase Order becomes effective upon signature by both parties and remains valid until the completion of all line items or 31 December 2025, whichever occurs first.
