The services encompass but are not limited to: assessment of functional capacity, development of individualized treatment plans, hand and upper-limb rehabilitation, cognitive-behavioral therapy for patients with neurological impairments, adaptive equipment training, and community reintegration programs. The Occupational Therapist must hold a valid registration with the Conselho Federal de Fisioterapia e Terapia Ocupacional (CREFITO) and maintain professional liability insurance throughout the duration of this engagement.

• 3.1 – This Purchase Order is governed by the laws of the Federative Republic of Brazil, specifically the Civil Code (Lei nº 10.406/2002) and applicable regulations of the Federal District of Brazil Brasília. Any disputes arising from this Purchase Order shall be resolved in the competent courts of Brasília – DF.
• 3.2 – The Occupational Therapist shall comply with all ethical standards established by the CREFITO and the Brazilian Ministry of Health. Failure to maintain professional registration or insurance shall result in immediate termination of this Purchase Order without penalty to HealthBridge Clinical Services.
• 3.3 – Payment shall be made in six (6) equal monthly installments of R$ 15.380,00 via bank transfer (TED/PIX) to the account designated by the Occupational Therapist. Payment is due within fifteen (15) business days following the end of each calendar month.
• 3.4 – The Occupational Therapist is engaged as an independent contractor and not as an employee of HealthBridge Clinical Services Ltda. The Occupational Therapist is solely responsible for the payment of all applicable taxes, including IRPF (Imposto de Renda Pessoa Física) and any contributions to the CREFITO.
• 3.5 – All patient records, clinical notes, and therapeutic documentation generated during the scope of this Purchase Order shall remain the property of HealthBridge Clinical Services and must be stored in compliance with the Brazilian General Data Protection Law (LGPD – Lei nº 13.709/2018).
• 3.6 – Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Occupational Therapist shall be compensated for all services rendered up to the effective date of termination.
• 3.7 – The Occupational Therapist shall maintain a minimum of 40 hours per week of availability at the HealthBridge Rehabilitation Center in Brazil Brasília, with the option to provide telehealth consultations for follow-up appointments as permitted by CREFITO regulations.
• 3.8 – This Purchase Order may be amended only by written addendum signed by both parties. Verbal modifications shall hold no legal validity.
