Scope of Work & Service Location

This Purchase Order authorizes the engagement of qualified Occupational Therapist professionals to deliver comprehensive therapeutic services across multiple clinical and community sites within Israel Jerusalem. All Occupational Therapist personnel assigned under this Purchase Order must hold a valid license issued by the Israeli Ministry of Health and possess a minimum of five (5) years of post-licensure clinical experience. The primary service delivery locations are situated in the central and eastern districts of Israel Jerusalem, including the Jerusalem Rehabilitation Center, the Children's Developmental Clinic, the Orthopedic Institute, and the Employment Integration Center. The Occupational Therapist team shall adhere to all clinical protocols established by the Jerusalem Regional Health & Rehabilitation Authority and comply with the standards set forth by the Israeli Occupational Therapy Association.

Terms and Conditions

Payment Terms: Net 45 days from the date of invoice submission. Payment shall be processed via bank transfer to the vendor's designated account in Israel Jerusalem. Invoices must reference this Purchase Order number (PO-JER-2025-04871) for processing.
Service Period: This Purchase Order covers a continuous service period of fifty-two (52) weeks commencing on 1 July 2025 and concluding on 30 June 2026, unless terminated earlier in accordance with the cancellation clause.
Performance Standards: The Occupational Therapist personnel must maintain a minimum patient satisfaction score of 85% as measured by quarterly surveys conducted by the Israel Jerusalem health authority. Failure to meet this threshold for two consecutive quarters shall constitute grounds for contract review.
