This Purchase Order is issued by Al-Nahda Medical & Rehabilitation Center to formally procure the professional services of a qualified Occupational Therapist to be deployed at our facility located in Kuwait Kuwait City. The Occupational Therapist shall be responsible for assessing, planning, and delivering evidence-based occupational therapy interventions to patients with neurological, orthopedic, developmental, and psychiatric conditions. The Occupational Therapist must hold a valid license issued by the Kuwait Ministry of Health and possess a minimum of five (5) years of post-qualification clinical experience in adult and pediatric rehabilitation settings. All services rendered under this Purchase Order shall be conducted exclusively within the premises of the facility in Kuwait Kuwait City, in full compliance with the regulations of the Kuwait Ministry of Health and the Kuwait Occupational Therapy Board.

- This Purchase Order constitutes a binding agreement between the Buyer and the Seller for the provision of Occupational Therapist services as described herein. All terms are governed by the laws of the State of Kuwait.
- The Occupational Therapist shall commence duties no later than 1 August 2025 at the Al-Nahda Medical & Rehabilitation Center facility in Kuwait Kuwait City. A probationary period of three (3) months shall apply, during which either party may terminate this Purchase Order with fourteen (14) days written notice.
- Payment shall be made in monthly installments of KWD 1,662.50 (one thousand six hundred sixty-two Kuwaiti Dinars and fifty nayeem) via bank transfer to the account designated by the Seller, due within thirty (30) days of the end of each calendar month.
- The Seller guarantees that the Occupational Therapist assigned under this Purchase Order holds a Bachelor's or Master's degree in Occupational Therapy from an accredited institution, a valid Kuwait Ministry of Health practice license, and current Basic Life Support (BLS) certification.
- All patient records, clinical notes, and treatment plans generated by the Occupational Therapist in Kuwait Kuwait City shall remain the exclusive property of the Buyer and shall be maintained in accordance with Kuwaiti data protection and patient confidentiality regulations.
- The Seller shall provide a replacement Occupational Therapist of equivalent or superior qualifications within ten (10) business days should the originally assigned therapist become unavailable for any reason, at no additional cost to the Buyer.
- Any modifications, amendments, or extensions to this Purchase Order must be documented in writing and signed by the authorized representatives of both parties. Verbal agreements shall hold no legal standing.
- In the event of a dispute arising from this Purchase Order, both parties agree to first attempt resolution through good-faith mediation in Kuwait Kuwait City. Should mediation fail, the matter shall be referred to the competent courts of the State of Kuwait.
- This Purchase Order is valid for a period of twelve (12) months from the date of commencement and may be renewed by mutual written agreement of both parties at least thirty (30) days prior to expiration.
- The Seller shall indemnify and hold harmless the Buyer against any claims, liabilities, or damages arising from the negligence, malpractice, or professional misconduct of the Occupational Therapist while performing duties under this Purchase Order in Kuwait Kuwait City.
By signing below, both parties acknowledge that they have read, understood, and agree to all terms and conditions set forth in this Purchase Order for the procurement of Occupational Therapist services in Kuwait Kuwait City.
