- This Purchase Order is valid for acceptance within fourteen (14) calendar days from the date of issue. Failure to accept within this period shall render this Purchase Order null and void.
- All services rendered by the Occupational Therapist shall be conducted in compliance with the Myanmar Health Professions Council regulations and the ethical code of practice for occupational therapy in Myanmar Yangon.
- Payment terms: Net 30 days from the date of invoice submission. Payment shall be made via bank transfer to the vendor's designated account in Myanmar Kyat (MMK). The total Purchase Order value of 32,100,000 MMK shall be disbursed in monthly installments for the Occupational Therapist service component and in a single lump sum upon delivery and acceptance of all physical goods.
- The vendor warrants that all equipment and materials supplied under this Purchase Order are new, unused, and free from defects. A minimum two-year warranty applies to all adaptive devices and assessment tools.
- Delivery of all physical items shall be made to the buyer's facility at 42 Strand Road, Sanchaung Township, Myanmar Yangon. The vendor is responsible for all transportation, insurance, and customs clearance (if applicable) costs within Myanmar Yangon.
- The Occupational Therapist engaged under this Purchase Order must hold a valid Myanmar Occupational Therapy Board license and a minimum of three (3) years of post-qualification clinical experience. Proof of licensure shall be submitted prior to the commencement of services.
- Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the buyer shall pay for all services rendered and goods delivered up to the termination date.
- All disputes arising from this Purchase Order shall be resolved through mediation in Myanmar Yangon in accordance with the laws of the Republic of the Union of Myanmar. The Myanmar Arbitration Centre shall serve as the arbitral body if mediation fails.
- The vendor shall maintain professional liability insurance with a minimum coverage of 50,000,000 MMK for the duration of the Occupational Therapist's engagement under this Purchase Order.
- Confidentiality: All patient records, clinical data, and proprietary treatment protocols accessed by the Occupational Therapist in the course of this Purchase Order shall remain the exclusive property of the buyer and shall not be disclosed to any third party without written consent.
