- Scope of Services: The Occupational Therapist engaged under this Purchase Order shall provide comprehensive occupational therapy services including but not limited to functional assessments, individualized treatment plans, direct therapeutic interventions, and progress documentation for all patients referred to the Peru Lima facility.
- Payment Terms: Payment shall be made within thirty (30) calendar days from the date of receipt of a valid invoice (factura) issued in accordance with Peruvian tax regulations. Payment will be processed via bank transfer to the account designated by the vendor in Peru Lima.
- Delivery and Installation: All physical items listed in this Purchase Order (equipment, workstation, materials) shall be delivered to the address in Peru Lima specified in the vendor information block. Installation and setup shall be completed within five (5) business days of delivery.
- Compliance: The Occupational Therapist must comply with all applicable Peruvian health regulations, data protection laws (Ley N° 29733 – Personal Data Protection Law), and the ethical code of the Peruvian College of Occupational Therapists in Peru Lima.
- Warranty: All equipment and materials supplied under this Purchase Order carry a minimum warranty of twelve (12) months from the date of delivery in Peru Lima. The vendor shall be responsible for any repairs or replacements within the warranty period.
- Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the Occupational Therapist shall complete all ongoing patient sessions and provide full clinical documentation before the effective termination date.
- Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through amicable negotiation. If unresolved, the matter shall be submitted to the competent courts of Peru Lima, Republic of Peru.
- Confidentiality: All patient information handled by the Occupational Therapist in the course of services under this Purchase Order shall be treated as strictly confidential in accordance with Peruvian medical ethics and privacy legislation applicable in Peru Lima.
