Description of Purchase – Occupational Therapist ServicesThis Purchase Order is issued by Al-Shifa Medical Group, Jeddah, to formally procure the professional services of a qualified Occupational Therapist for deployment at the Al-Shifa Rehabilitation Center located in Jeddah, Saudi Arabia. The Occupational Therapist shall provide comprehensive therapeutic interventions, patient assessments, treatment plan development, and ongoing clinical supervision in accordance with the standards set forth by the Saudi Commission for Health Specialties (SCFHS) and the Ministry of Health, Saudi Arabia. The Occupational Therapist must hold a valid SCFHS license, a recognized Bachelor's or Master's degree in Occupational Therapy from an accredited institution, and a minimum of five (5) years of post-qualification clinical experience in pediatric and adult rehabilitation settings.

1. This Purchase Order is governed by the laws of the Kingdom of Saudi Arabia and the commercial regulations applicable in Jeddah, Saudi Arabia. Any disputes arising from this Purchase Order shall be resolved through the competent courts in Jeddah or through arbitration as stipulated under Saudi commercial law.
2. The Occupational Therapist engaged under this Purchase Order must maintain a valid SCFHS professional license throughout the entire contract period. The vendor shall provide proof of licensure prior to the Occupational Therapist's first day of service at the Jeddah facility.
3. The Occupational Therapist shall comply with all data protection and patient confidentiality requirements as mandated by the Saudi Ministry of Health and the Personal Data Protection Law of Saudi Arabia. All patient records generated in Jeddah shall be stored in accordance with local regulatory standards.
4. Payment shall be processed via bank transfer to the vendor's designated account in Saudi Riyal (SAR) within thirty (30) calendar days of receipt of a valid tax invoice. Late payments shall incur a penalty of 1.5% per month as per Saudi commercial practice.
5. The vendor warrants that the Occupational Therapist assigned to this Purchase Order shall not be concurrently engaged with any competing healthcare facility within a 10-kilometer radius of the Jeddah service location during the contract term.
6. Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, the vendor shall ensure a minimum of two (2) weeks of transition support for the Occupational Therapist's patient caseload at the Jeddah facility.
7. The Occupational Therapist shall be subject to the internal policies, health and safety protocols, and code of conduct of Al-Shifa Medical Group, Jeddah, as well as all applicable Saudi labor laws and regulations.
Important Notes: This Purchase Order is specific to the deployment of an Occupational Therapist at the Al-Shifa Rehabilitation Center in Jeddah, Saudi Arabia. All services described herein are to be rendered exclusively within the Jeddah metropolitan area unless otherwise agreed in writing. The vendor acknowledges that the Occupational Therapist must be physically present at the Jeddah facility for all scheduled clinical hours. Any substitution of the Occupational Therapist requires prior written approval from the Chief Procurement Officer of Al-Shifa Medical Group.
