This Purchase Order is issued to procure the professional services of a qualified Occupational Therapist to be deployed and rendered at the designated rehabilitation facilities within Tanzania Dar es Salaam. The Occupational Therapist shall provide comprehensive assessment, intervention planning, and hands-on therapeutic services to patients with physical, cognitive, and developmental disabilities. The scope of work encompasses the following deliverables:

- Qualification Requirements: The Occupational Therapist engaged under this Purchase Order must hold a Bachelor's or Master's degree in Occupational Therapy from a recognized institution, be registered with the Allied Health Professions Council of Tanzania, and possess valid professional indemnity insurance covering all services rendered in Tanzania Dar es Salaam.
- Place of Service: All services shall be performed exclusively within the administrative boundaries of Tanzania Dar es Salaam, with primary duty station at the Dar es Salaam Rehabilitation Centre, Samora Avenue. The Occupational Therapist may be required to travel to district hospitals within the region for training and outreach activities.
- Payment Schedule: Payment shall be made in monthly installments within thirty (30) calendar days of receipt of a valid invoice. The first payment shall be due upon successful onboarding and verification of the Occupational Therapist's credentials by the Regional Health Bureau.
- Performance Standards: The Occupational Therapist shall adhere to the clinical guidelines issued by the Ministry of Health, Community Development, Gender, Elderly and Children of the United Republic of Tanzania. Failure to meet documented performance benchmarks for two consecutive months shall constitute grounds for contract termination with thirty (30) days' written notice.
- Confidentiality: The Occupational Therapist shall maintain strict confidentiality of all patient records and clinical data in accordance with the Tanzania Data Protection Act, 2022. No patient information shall be disclosed to third parties without written consent.
- Termination: Either party may terminate this Purchase Order with sixty (60) days' written notice. In the event of material breach, the non-breaching party may terminate immediately upon written notice. All outstanding invoices for services rendered prior to termination shall be settled within thirty (30) days.
- Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the United Republic of Tanzania. Any disputes arising herefrom shall be resolved through arbitration in Tanzania Dar es Salaam in accordance with the Arbitration Act, Cap. 20 of the Laws of Tanzania.
- Force Majeure: Neither party shall be liable for failure to perform obligations due to events beyond reasonable control, including but not limited to natural disasters, epidemics, or government-imposed restrictions affecting operations in Tanzania Dar es Salaam.
- Intellectual Property: All treatment protocols, training materials, and clinical documentation developed specifically for this engagement shall remain the property of the Dar es Salaam Regional Health Bureau upon completion of the contract period.
- Compliance: The Supplier warrants that the Occupational Therapist shall comply with all applicable Tanzanian labor laws, immigration regulations (if applicable), and professional ethical codes throughout the duration of this Purchase Order.
This Purchase Order is valid for a period of ninety (90) days from the date of issue. The Supplier is required to confirm acceptance of all terms and conditions herein by signing and returning a copy of this document within fourteen (14) calendar days of receipt. Failure to respond within the stipulated period shall render this Purchase Order null and void.
