This Purchase Order is issued by the Tashkent Regional Rehabilitation and Occupational Therapy Center (hereinafter referred to as the "Purchasing Authority") to procure the professional services of a qualified Occupational Therapist, along with associated clinical equipment, training materials, and administrative support, for delivery and implementation within the Tashkent, Uzbekistan service area. This Purchase Order governs the engagement, compensation, deliverables, and compliance obligations of the Occupational Therapist and the supplying entity for a contract period of twelve (12) months, commencing on 1 August 2025 and concluding on 31 July 2026.
The Occupational Therapist engaged under this Purchase Order shall provide comprehensive therapeutic interventions, patient assessments, treatment plan development, and multidisciplinary coordination services to patients with neurological, orthopedic, developmental, and age-related functional impairments at the Tashkent Regional Rehabilitation and Occupational Therapy Center and its affiliated satellite clinics in the Tashkent metropolitan area.

Approximate USD equivalent at current exchange rate: ~$31,781.00. All payments shall be processed in Uzbekistan Som (UZS) via bank transfer to the account specified in Section 1.
4.1 Compliance with Uzbekistan Law. The Occupational Therapist engaged under this Purchase Order shall hold a valid license issued by the Ministry of Health of the Republic of Uzbekistan and shall comply with all applicable national and local regulations governing healthcare practice in Tashkent, Uzbekistan. The supplier shall ensure that the Occupational Therapist's credentials, including a recognized degree in Occupational Therapy and any required post-graduate certifications, are verified and maintained throughout the contract period.
4.2 Service Delivery Location. All therapeutic services, patient interactions, and clinical documentation under this Purchase Order shall be conducted at the Tashkent Regional Rehabilitation and Occupational Therapy Center, 142 Amir Temur Avenue, Tashkent, Uzbekistan, or at approved satellite locations within the Tashkent city limits as designated by the Purchasing Authority.
4.3 Payment Schedule. Payment for this Purchase Order shall be made in four (4) quarterly installments, due within thirty (30) calendar days following the end of each quarter. The first installment of 99,316,000 UZS shall be due by 30 September 2025. Late payments shall incur a penalty of 0.5% per calendar day as stipulated under Uzbekistan commercial law.
4.4 Performance Standards. The Occupational Therapist shall conduct a minimum of twenty (20) patient sessions per week, maintain complete and accurate clinical records within forty-eight (48) hours of each session, and submit monthly progress reports to the Purchasing Authority's Clinical Director. Failure to meet these performance standards for two consecutive months shall constitute grounds for termination of this Purchase Order with thirty (30) days' written notice.
4.5 Confidentiality and Data Protection. The Occupational Therapist and the supplier shall adhere to the Law of the Republic of Uzbekistan on Personal Data Protection and all institutional confidentiality policies. Patient records, treatment plans, and any proprietary methodologies shall not be disclosed to third parties without written consent from the Purchasing Authority.
4.6 Termination. Either party may terminate this Purchase Order with sixty (60) days' written notice. In the event of material breach, termination may be effected with fourteen (14) days' notice. Upon termination, the supplier shall return all equipment, materials, and documentation provided under this Purchase Order to the Tashkent facility within ten (10) business days.
4.7 Dispute Resolution. Any disputes arising from this Purchase Order shall first be subject to good-faith negotiation. If unresolved within thirty (30) days, the matter shall be referred to the Arbitration Center of the Chamber of Commerce and Industry of the Republic of Uzbekistan in Tashkent for binding arbitration.
This Purchase Order becomes effective upon signature by both parties. The supplier acknowledges receipt of this Purchase Order and agrees to furnish the Occupational Therapist services, equipment, and support as described herein. All modifications to this Purchase Order must be made in writing and signed by authorized representatives of both parties.
