  1. This Purchase Order constitutes a binding agreement between the Kabul Medical Procurement Authority and the named Ophthalmologist for the provision of ophthalmic medical services within the city of Afghanistan Kabul. Both parties acknowledge and accept all terms stated herein.
  2. The Ophthalmologist shall provide all services at the Kabul Central Eye Hospital, located in Darulaman District, Afghanistan Kabul, during the period specified in this Purchase Order. The Ophthalmologist must maintain valid medical licensure and professional registration throughout the engagement period.
  3. Payment for services rendered under this Purchase Order shall be made in Afghan Afghani (AFN) via bank transfer to the account designated by the Ophthalmologist. Invoices must be submitted monthly, no later than the fifth business day of the following month, and payment shall be processed within thirty (30) calendar days of invoice receipt.
  4. The Ophthalmologist is required to maintain comprehensive medical records for all patients treated in Afghanistan Kabul in accordance with the data protection and patient confidentiality regulations of the Republic of Afghanistan. All records must be submitted to the Kabul Medical Procurement Authority at the conclusion of each service period.
  5. Any modification, amendment, or cancellation of this Purchase Order must be made in writing and signed by both parties. Verbal agreements regarding the scope of the Ophthalmologist's duties in Afghanistan Kabul shall not be considered valid amendments to this document.
  6. The Ophthalmologist shall be responsible for all medical malpractice insurance coverage during the term of this Purchase Order. Proof of insurance must be provided prior to the commencement of services in Afghanistan Kabul.
  7. In the event of force majeure, including but not limited to security disruptions, natural disasters, or government-mandated closures affecting Afghanistan Kabul, either party may suspend obligations under this Purchase Order with written notice to the other party within forty-eight (48) hours.
  8. All disputes arising from this Purchase Order shall be resolved through mediation in Afghanistan Kabul before resorting to litigation. The governing law shall be the civil and commercial law of the Islamic Republic of Afghanistan.
  9. The Ophthalmologist agrees to comply with all ethical standards of medical practice as defined by the Afghan Medical Association and to report any conflicts of interest to the Kabul Medical Procurement Authority in a timely manner.

By signing below, both parties confirm that this Purchase Order for the services of the Ophthalmologist in Afghanistan Kabul has been reviewed, understood, and accepted in its entirety. This document is valid for a period of six (6) months from the date of issue, unless renewed or extended in writing by both parties.
