1. Scope of Purchase Order: This Purchase Order is issued by Clínica Ocular Andina SpA, a registered ophthalmology clinic located in Chile Santiago, for the procurement of specialized ophthalmological equipment, surgical instruments, consumable supplies, and professional services. All items listed herein are intended to support the clinical practice of the Ophthalmologist, Dr. Carolina Reyes, and her associated medical staff at the clinic's primary facility in the Providencia commune of Santiago, Chile.
2. Delivery and Installation: The supplier, MedEquip Chile Ltda., shall deliver all items listed in this Purchase Order to the Ophthalmologist's clinic at Av. Providencia 1234, Of. 501, Providencia, Santiago, Chile. Delivery shall be made within the timeframes specified in the "Delivery" column. All equipment requiring installation, calibration, or configuration for the Ophthalmologist's use shall be installed by certified technicians from the supplier at no additional cost. The supplier acknowledges that the Chile Santiago location operates within the Metropolitan Region and that all deliveries must comply with local municipal regulations regarding large equipment transport.
3. Payment Terms: Payment for this Purchase Order shall be made in Chilean Pesos (CLP) via electronic bank transfer (transferencia electrónica) to the supplier's designated account. A 30% advance payment is due upon acceptance of this Purchase Order. The remaining 70% shall be paid within 30 calendar days following confirmed delivery, installation, and successful calibration of all Ophthalmologist equipment. 4.
