Terms and Conditions of This Purchase Order
- This Purchase Order is valid for a period of thirty (30) calendar days from the date of issue. The Ophthalmologist, Dr. Ahmed Raza Khan, must confirm acceptance in writing within fourteen (14) days of receipt of this Purchase Order.
- All medical equipment and consumables listed in this Purchase Order must be delivered to the Islamabad Vision Care Centre, F-8 Markaz, Islamabad, Pakistan, no later than 30 June 2025. Late delivery shall incur a penalty of 0.5% of the total Purchase Order value per day of delay.
- The Ophthalmologist is required to inspect all items upon delivery and sign the delivery acknowledgment form. Any discrepancies, damages, or missing items must be reported to the Islamabad Medical Procurement Authority within forty-eight (48) hours of delivery.
- Payment shall be processed via bank transfer to the supplier's designated account within thirty (30) days of successful delivery and acceptance. The Ophthalmologist's clinic shall provide a valid Sales Tax Invoice (STI) as per the Federal Board of Revenue (FBR), Pakistan, regulations.
- All imported medical devices and equipment must carry valid DRAP (Drug Regulatory Authority of Pakistan) registration certificates and must comply with the Pakistan Medical Device Regulations 2023. The Ophthalmologist shall ensure that all equipment is registered with the Islamabad Health Department prior to clinical use.
- The supplier shall provide a minimum two-year warranty on all capital equipment (items 01, 02, 04, 07, 08, and 10) and shall include annual maintenance and calibration services as part of the Purchase Order agreement.
- This Purchase Order is governed by the laws of the Islamic Republic of Pakistan. Any disputes arising from this Purchase Order shall be resolved through arbitration in Islamabad, Pakistan, in accordance with the Arbitration Act 1940.
- The Ophthalmologist agrees to use all procured items exclusively for patient care at the Islamabad Vision Care Centre and shall not resell, transfer, or sublet any equipment covered under this Purchase Order without prior written consent from the Islamabad Medical Procurement Authority.
- All data related to this Purchase Order, including patient treatment records generated using the procured equipment, shall be maintained in compliance with the Pakistan Personal Data Protection Bill and stored within the jurisdiction of Islamabad, Pakistan.
