4.1 This Purchase Order is governed by the laws of the State of Qatar and the commercial regulations applicable in Qatar Doha. Any disputes arising from this Purchase Order shall be resolved through the competent courts in Doha, Qatar.
4.2 The Ophthalmologist engaged under this Purchase Order must hold a valid medical license issued by the Qatar Health Regulatory Authority (QPHRA) and must maintain professional indemnity insurance with a minimum coverage of QAR 2,000,000 throughout the contract duration.
4.3 All ophthalmic equipment procured under this Purchase Order must comply with the International Organization for Standardization (ISO) 13485 medical device standards and must be registered with the Ministry of Public Health, Qatar, prior to installation in Qatar Doha facilities.
4.4 Payment terms: 30% advance upon acceptance of this Purchase Order, 50% upon delivery and successful installation of all equipment in Doha, and 20% upon completion of the 90-day performance warranty period. Payments shall be made via bank transfer to the account specified by the Ophthalmologist and equipment suppliers.
4.5 The Ophthalmologist shall submit monthly clinical activity reports to the Ophthalmology Department Head at Hamad Medical Corporation, Doha, detailing patient volumes, surgical outcomes, and any adverse events related to ophthalmic procedures performed.
4.6 This Purchase Order is valid for acceptance within fourteen (14) calendar days from the date of issue. Failure to confirm acceptance within this period shall render this Purchase Order null and void.
4.7 The supplier and the Ophthalmologist shall bear all customs duties, import taxes, and logistics costs associated with the delivery of ophthalmic equipment to the designated facility in Qatar Doha.
4.8 Any modification to the scope, pricing, or delivery schedule of this Purchase Order must be documented in a written amendment signed by both parties and countersigned by the Procurement Director, Hamad Medical Corporation, Doha.
5. DELIVERY & INSTALLATIONAll equipment items listed in Section 3 shall be delivered to the Ophthalmology Department, Hamad Medical Corporation, P.O. Box 20153, Doha, Qatar. The Ophthalmologist shall be present during the installation and calibration of all ophthalmic devices to ensure proper integration into the clinical workflow in Qatar Doha. A joint acceptance inspection shall be conducted by the Buyer's biomedical engineering team and the Ophthalmologist before final sign-off.
