- Governing Jurisdiction: This Purchase Order shall be governed by and construed in accordance with the laws of the State of California, United States. All disputes arising from this Purchase Order relating to Ophthalmologist services or equipment shall be resolved in the Superior Court of Los Angeles County, United States Los Angeles.
- Delivery and Installation: All equipment items listed in this Purchase Order shall be delivered to the buyer's facility at 4500 Wilshire Boulevard, United States Los Angeles, CA 90012, no later than July 15, 2025. Installation, calibration, and operator training for the Ophthalmologist staff shall be completed within five (5) business days of delivery.
