All amounts are quoted in United States Dollars (USD) as per the prevailing exchange rate approved by the Reserve Bank of Zimbabwe. Payment shall be processed through the hospital's central accounts office in Harare, Zimbabwe, via electronic funds transfer (EFT) to the supplier's designated bank account.

The following terms and conditions govern this Purchase Order and the engagement of the Ophthalmologist:

  1. The Ophthalmologist shall maintain valid professional registration with the Zimbabwe Medical and Dental Council and shall provide a copy of the current registration certificate prior to commencing services under this Purchase Order.
  2. All clinical services rendered by the Ophthalmologist shall adhere to the clinical guidelines issued by the Zimbabwe National Eye Health Strategy and the World Health Organization (WHO) standards for ophthalmic care.
  3. The Ophthalmologist shall maintain adequate professional indemnity insurance coverage of no less than USD 500,000 for the duration of this Purchase Order.
  4. Confidentiality of all patient records and medical data shall be strictly observed in accordance with the Zimbabwe Data Protection Act (Chapter 24:08) and hospital privacy policies.
  5. Delivery of all equipment and consumables listed under this Purchase Order shall be completed at the Parirenyatwa General Hospital premises in Zimbabwe Harare no later than the delivery deadline stated above.
  6. Any variation to the scope of services or quantities under this Purchase Order must be approved in writing by the Hospital Procurement Committee and the Chief Medical Officer before execution.
  7. Disputes arising from this Purchase Order shall be resolved through mediation in accordance with the laws of the Republic of Zimbabwe, with jurisdiction vested in the courts of Harare, Zimbabwe.
  8. The Ophthalmologist shall submit monthly service reports detailing patient volumes, surgical outcomes, and any adverse events to the Hospital Administration within five (5) business days of the end of each calendar month.

This Purchase Order is authorised and issued on behalf of Parirenyatwa General Hospital, Zimbabwe Harare, by the undersigned. The Ophthalmologist and/or the Harare Vision & Eye Care Centre shall confirm acceptance of this Purchase Order by signing and returning a copy within five (5) working days of receipt. Failure to confirm acceptance within the stipulated period shall be taken as a declination of this Purchase Order.
