- This Purchase Order is issued by ClearVision Optometry Group, a registered Optometrist practice operating in Australia Melbourne, and constitutes a binding agreement upon acceptance by the supplier.
- All goods must be delivered to the Optometrist practice premises at 42 Collins Street, Melbourne VIC 3000, Australia Melbourne, no later than 28 June 2025. Late delivery will incur a penalty of 1.5% of the total order value per calendar day.
