Note to Supplier: All items listed in this Purchase Order are intended for use by a registered Optometrist at our practice in Ghana Accra. The supplier is required to ensure that all optical equipment and lenses comply with the standards set by the Ghana Optometric Council and the Food and Drugs Authority (FDA) of Ghana. All imported lenses and frames must carry valid CE marking or equivalent certification. Delivery must be made to our premises in Osu, Accra, and all equipment requiring installation must be set up and calibrated by the supplier's technical team before the required delivery date.Terms and Conditions of this Purchase Order
- This Purchase Order is valid for acceptance within seven (7) calendar days from the date of issue. Failure to confirm acceptance within this period shall render this Purchase Order null and void.
- All goods must be delivered to the specified address in Ghana Accra no later than 28 June 2025. Late delivery shall attract a penalty of 1% of the total Purchase Order value per day of delay, up to a maximum of 10%.
- Payment shall be made via bank transfer to the supplier's designated account in Ghana within thirty (30) days of receipt of a valid tax invoice, subject to satisfactory inspection and acceptance of all goods by the Optometrist in charge.
- All equipment carries a minimum warranty period of twelve (12) months from the date of delivery, unless otherwise stated in the line item remarks. Warranty claims must be directed to the supplier's service centre in Accra, Ghana.
- The supplier warrants that all optical products supplied under this Purchase Order are new, unused, and free from defects in materials and workmanship. Any item found to be defective upon inspection by the Optometrist shall be replaced at the supplier's sole cost within five (5) business days.
- This Purchase Order is governed by the laws of the Republic of Ghana. Any disputes arising from this Purchase Order shall be resolved through arbitration in Accra, Ghana, in accordance with the Arbitration Act, 1998 (Act 562).
- The supplier shall provide a detailed packing list and all relevant import documentation (where applicable) to facilitate smooth customs clearance for any items sourced internationally and delivered to our Ghana Accra facility.
- Partial deliveries are not permitted unless expressly agreed in writing by the purchasing Optometrist. All items must arrive in a single consignment to the specified Ghana Accra address.
- This Purchase Order does not constitute a guarantee of future orders. The Accra Vision Care & Optometry Centre reserves the right to place additional Purchase Orders with the supplier based on the performance and quality of goods delivered under this order.
