Terms and Conditions of This Purchase Order
- This Purchase Order is valid for acceptance within fourteen (14) calendar days from the date of issue. Failure to confirm acceptance by the stated deadline shall render this Purchase Order null and void.
- All items listed in this Purchase Order are intended for use in a licensed Optometrist practice located in Israel Jerusalem. The supplier acknowledges that the goods will be used in a regulated healthcare environment and must meet all applicable Israeli medical and optical standards.
- Delivery shall be made to the specified address in Israel Jerusalem between 08:00 and 16:00 on any business day (Monday through Friday). The supplier is responsible for all transportation costs, insurance, and risk of loss until the goods are received and signed for by the designated Optometrist representative at the Jerusalem facility.
- Payment shall be made within thirty (30) days of receipt of a valid tax invoice (Faskal) issued by the supplier, in accordance with Israeli tax law. Payment shall be made via bank transfer to the supplier's designated account in Israeli New Shekels (ILS).
- All equipment must be accompanied by manufacturer warranties, user manuals in Hebrew and English, and certificates of conformity. The Optometrist practice reserves the right to reject any item that does not meet the specifications outlined in this Purchase Order or that fails to comply with Israeli regulatory requirements.
- The supplier warrants that all optical lenses, frames, and contact lenses supplied under this Purchase Order are free from defects in materials and workmanship for a period of no less than twenty-four (24) months from the date of delivery to Israel Jerusalem.
- In the event of a dispute arising from this Purchase Order, the parties agree to first attempt resolution through good-faith negotiation. Should negotiation fail, the dispute shall be submitted to the competent courts in Jerusalem, Israel, in accordance with Israeli law.
- This Purchase Order is governed by and shall be construed in accordance with the laws of the State of Israel. Any amendments to this document must be made in writing and signed by both parties.
- The supplier shall maintain full insurance coverage for the goods in transit from Tel Aviv to the delivery location in Israel Jerusalem. The supplier's insurance policy number and provider details must be provided upon request.
