- This Purchase Order is issued by Kyoto Vision Care Optometrist Clinic, a licensed Optometrist practice registered in the Japan Kyoto metropolitan area, and constitutes a binding procurement request for all items listed above.
- All equipment and supplies must comply with the Japan Medical Device Act (MDA) and meet the standards set by the Japan Optometrists Association for use in a licensed Optometrist clinic within Japan Kyoto.
