- This Purchase Order is issued by ClearVision Optometry & Eye Care Centre, a registered Optometrist practice operating in Nepal Kathmandu, and is binding upon acceptance by the supplier.
- All equipment and materials listed in this Purchase Order must be new, unused, and in original manufacturer packaging. The Optometrist reserves the right to reject any item that does not meet the specified standards.
- Delivery of all items shall be made to the Optometrist's clinic at New Baneshwor, Kathmandu, Nepal, no later than 30 June 2025. The supplier is responsible for all transportation, handling, and installation of diagnostic equipment within Nepal Kathmandu city limits.
- Payment terms: 40% advance payment upon confirmation of this Purchase Order, and 60% balance payment within 15 days of successful delivery, installation, and acceptance by the Optometrist.
- All diagnostic instruments (Items 01–06 and 10) shall carry a minimum of 24-month manufacturer warranty. The Optometrist shall be entitled to on-site repair or replacement within 72 hours of reporting a defect in Nepal Kathmandu.
- The supplier shall provide comprehensive training for the Optometrist and two (2) assistant staff members on the operation and maintenance of all delivered equipment. Training sessions shall be conducted at the clinic in Nepal Kathmandu at no additional cost.
- Optical lenses and frames (Items 07–09) must comply with the Nepal Standards and Metrology Department (NSMD) regulations for optical products sold in Nepal Kathmandu. Certificates of conformity must accompany each batch.
- Any delay in delivery beyond the stipulated date shall incur a penalty of 0.5% of the total Purchase Order value per day of delay, up to a maximum of 10%.
- This Purchase Order is governed by the laws of the Kingdom of Nepal. Any disputes arising from this transaction shall be resolved through the competent courts in Kathmandu, Nepal.
- The Optometrist practice in Nepal Kathmandu reserves the right to modify, cancel, or amend this Purchase Order in writing prior to the supplier's dispatch of goods. No verbal modifications shall be valid.
- All invoices must reference this Purchase Order number (PO/2025/KTM/00472) and include the supplier's VAT registration number for processing by the Optometrist's accounting department.
