Terms and Conditions of this Purchase Order
- This Purchase Order is issued by Wellington Vision Care Optometry Ltd., a duly registered Optometrist practice in New Zealand Wellington, and constitutes a binding offer to purchase the goods listed above upon acceptance by the supplier.
- All goods must be delivered to our Optometrist practice premises at 142 Lambton Quay, Te Aro, Wellington 6011, New Zealand, no later than the required delivery date stated above. Late delivery will incur a penalty of 1.5% of the Purchase Order value per calendar day of delay.
- The supplier warrants that all optical equipment and supplies listed in this Purchase Order are new, unused, and comply with the New Zealand Standards (NZS) for medical and optical devices, as well as the requirements set forth by the New Zealand Optometrists and Dispensing Opticians (NZODO) regulatory body.
- Payment of this Purchase Order shall be made within thirty (30) calendar days of the date of receipt of a valid tax invoice from the supplier, via bank transfer to the account nominated by the supplier. All amounts are quoted in New Zealand Dollars (NZD) and are inclusive of applicable Goods and Services Tax (GST) at the current New Zealand rate of 15%.
- All equipment supplied under this Purchase Order for Optometrist clinical use must include a minimum two-year manufacturer's warranty, with on-site repair service available within the New Zealand Wellington metropolitan area within forty-eight (48) hours of a service request.
- The supplier shall provide full installation, calibration, and staff training for all capital equipment items (Lines 1, 2, 4, 5, and 10) at our Optometrist practice in New Zealand Wellington at no additional cost beyond the Purchase Order total.
- Any goods found to be defective, damaged in transit, or not conforming to the specifications in this Purchase Order must be replaced or refunded within fourteen (14) days of written notice from our Optometrist practice management.
- This Purchase Order is governed by the laws of New Zealand. Any disputes arising from this Purchase Order shall be resolved through the District Court of Wellington, New Zealand, or by mediation under the New Zealand Dispute Resolution Service.
- The supplier acknowledges that the goods purchased under this Purchase Order will be used in the provision of Optometrist services to the public of New Zealand Wellington and must meet all applicable health, safety, and professional standards required for clinical optical practice in New Zealand.
- This Purchase Order may not be assigned or transferred without the prior written consent of Wellington Vision Care Optometry Ltd. Any amendments to this Purchase Order must be made in writing and signed by both parties.
