- This Purchase Order is issued by ClearVision Optometry Center, a licensed Optometrist practice registered with the Ministry of Public Health, State of Qatar, and is valid for acceptance within five (5) business days from the date of issue.
- All equipment and consumables listed herein are intended for use by the licensed Optometrist team at our Doha, Qatar clinic located at The Pearl – Qanat Quartier. The supplier shall ensure that all diagnostic instruments comply with the Qatar Standards and Metrology Organization (QSMO) regulations and carry valid CE or equivalent certification.
- Delivery shall be made to the specified address in Doha, Qatar, no later than 28 June 2025. The supplier is responsible for all customs clearance, import duties, and local logistics within the State of Qatar. Any delays attributable to the supplier will incur a penalty of 1% of the total Purchase Order value per day of delay.
- Payment shall be processed via bank transfer to the supplier's designated account in Qatari Riyals (QAR) within thirty (30) calendar days of receipt of a valid tax invoice and confirmed delivery. The Purchase Order number PO-QD-2025-00472 must be referenced on all invoices.
- All optical equipment is subject to a minimum two-year manufacturer's warranty. The Optometrist team at our clinic will conduct a full acceptance inspection upon delivery. Any items found defective, damaged, or non-conforming to the specifications outlined in this Purchase Order must be replaced within seven (7) business days at the supplier's sole expense.
- The supplier warrants that all products are new, unused, and free from any encumbrance. Contact lenses and optical lenses must carry a minimum expiry date of eighteen (18) months from the date of delivery to ensure compliance with Qatar's pharmaceutical and medical device regulations.
- This Purchase Order is governed by the laws of the State of Qatar. Any disputes arising from this transaction shall be resolved through the competent courts in Doha, Qatar. Both parties agree to attempt amicable resolution before initiating any legal proceedings.
- The supplier acknowledges that this Purchase Order supports the operations of a registered Optometrist facility serving the community of Doha, Qatar, and agrees to maintain strict confidentiality regarding patient-related data, clinic operations, and proprietary business information exchanged during the course of this transaction.
