- This Purchase Order is issued by Anadolu Vision Optometry Center, a licensed Optometrist practice operating in Turkey Istanbul, and constitutes a binding agreement upon acceptance by the supplier.
- All equipment and supplies listed herein are intended for use by a certified Optometrist and must comply with the Turkish Ministry of Health (Sağlık Bakanlığı) regulations governing optical diagnostic and dispensing equipment.
- Delivery of all items shall be made to the buyer's premises located in Turkey Istanbul, Şişli district, no later than 30 June 2025. The supplier is responsible for all transportation, insurance, and installation costs within Turkey Istanbul.
- Payment shall be made via bank transfer to the supplier's designated account within thirty (30) calendar days of the invoice date, in Turkish Lira (TRY), in accordance with the terms stated in this Purchase Order.
- All diagnostic instruments supplied under this Purchase Order must carry a minimum two-year manufacturer warranty, with on-site service coverage available throughout Turkey Istanbul and its metropolitan area.
- The Optometrist practice requires that all equipment be CE-marked and, where applicable, bear the Turkish CE (TSE) conformity mark. The supplier shall provide all relevant conformity certificates and user manuals in both Turkish and English.
- Any deviation from the specifications listed in this Purchase Order must be approved in writing by the Lead Optometrist, Dr. Elif Kaya, prior to shipment. Unauthorized substitutions will result in rejection of the affected items at the supplier's expense.
- The supplier shall provide a comprehensive training session for the Optometrist and assisting staff at the Turkey Istanbul clinic upon installation of all major diagnostic equipment. Training shall cover operation, maintenance, and basic troubleshooting.
- This Purchase Order is governed by the laws of the Republic of Turkey. Any disputes arising from this agreement shall be subject to the exclusive jurisdiction of the courts in Turkey Istanbul.
- The supplier acknowledges that the equipment purchased under this Purchase Order will be used exclusively in the practice of a licensed Optometrist and shall not be resold or transferred without prior written consent from the buyer.
