Terms and Conditions of this Purchase Order
- This Purchase Order is issued by ClearSight Optometry Ltd., a registered Optometrist practice in United Kingdom London, and constitutes a binding order for the goods and services described herein. Acceptance of this Purchase Order by the supplier shall be deemed by written confirmation within five (5) working days of receipt.
- All goods must comply with the relevant United Kingdom regulations, including the Medical Devices Regulations 2002 (as amended) and the General Product Safety Regulations 2005. All optical equipment supplied to this Optometrist practice must carry valid CE/UKCA marking and be accompanied by full calibration and compliance documentation.
- Delivery of all items shall be made to the Optometrist practice premises at 142 Regent Street, London, W1B 5JD, United Kingdom, between 09:00 and 16:00 on the stated delivery date. The supplier shall provide 48 hours' written notice prior to delivery of any capital equipment exceeding £5,000.00 in value.
- Payment shall be made by BACS transfer to the supplier's nominated bank account within thirty (30) calendar days of receipt of a valid tax invoice. The invoice must reference this Purchase Order number (PO-2025-00487) and include the supplier's VAT registration number for United Kingdom London compliance.
- All equipment supplied to this Optometrist practice shall carry a minimum manufacturer's warranty of two (2) years from the date of installation. Any defects discovered within the warranty period shall be repaired or replaced at the supplier's sole cost, including travel to London, United Kingdom.
- The supplier warrants that all lenses, coatings, and consumables supplied are new, unused, and of the highest quality suitable for an Optometrist dispensing practice serving patients across London, United Kingdom. Substandard or expired products will be rejected at the point of delivery.
- Installation and calibration of all capital equipment (items 01, 02, 10, and 11) shall be carried out by the supplier's certified engineers at the Optometrist's London, United Kingdom premises. A full training session for the Optometrist clinical team shall be provided at no additional cost.
- This Purchase Order is governed by the laws of England and Wales. Any disputes arising from this Purchase Order shall be subject to the exclusive jurisdiction of the courts of London, United Kingdom.
- The supplier shall maintain adequate public liability insurance (minimum £10,000,000) and professional indemnity insurance throughout the duration of this Purchase Order and for a period of two years thereafter.
- ClearSight Optometry Ltd. reserves the right to amend, cancel, or partially reject this Purchase Order in writing prior to the delivery date. Any such amendments shall be agreed in writing by both parties.
