- This Purchase Order is issued by Kariakoo Smile Orthodontic Centre, a registered Orthodontist practice operating in Tanzania Dar es Salaam, and constitutes a binding procurement request for the orthodontic equipment and materials listed above.
- The supplier shall deliver all items to the Orthodontist's clinic at Plot 78, Sam Nujoma Street, Kariakoo, Tanzania Dar es Salaam, no later than 28 June 2025. All delivery, customs clearance, and import duties within Tanzania Dar es Salaam shall be the responsibility of the supplier under DDP terms.
- Payment shall be made within thirty (30) calendar days of the date of invoice, via bank transfer to the supplier's designated account. The Orthodontist's practice in Tanzania Dar es Salaam shall issue payment through the National Bank of Commerce, Dar es Salaam branch.
- All orthodontic equipment supplied under this Purchase Order must comply with the Tanzania Food and Drug Authority (TFDA) registration requirements and the Tanzania Medical Board (TMB) standards for clinical devices used by an Orthodontist in Tanzania Dar es Salaam.
- The supplier warrants that all items are new, unused, and free from defects. The Orthodontist's practice in Tanzania Dar es Salaam reserves the right to reject any non-conforming goods within fourteen (14) days of delivery.
- Installation and calibration of the intraoral X-ray imaging system and CBCT software shall be performed by the supplier's certified technician at the Orthodontist's clinic in Tanzania Dar es Salaam, at no additional cost beyond the delivery fee stated in this Purchase Order.
- A two-year warranty is provided on all capital equipment (Items 05 and 10). Consumable items (Items 01–04, 06–09) carry a standard 90-day manufacturer's warranty against manufacturing defects.
- This Purchase Order is governed by the laws of the United Republic of Tanzania. Any disputes arising from this Purchase Order shall be resolved through arbitration in Tanzania Dar es Salaam in accordance with the Tanzania Arbitration Act, Cap. 20.
- The Orthodontist's practice in Tanzania Dar es Salaam may amend or cancel this Purchase Order in writing prior to the supplier's confirmation of shipment. Any amendments must be countersigned by both parties.
- All communications regarding this Purchase Order shall be directed to the procurement officer of the Orthodontist's practice at the email and phone number listed above, or to the supplier's regional sales director in Mombasa, Kenya.
