This Purchase Order is issued by the Manila Emergency Medical Services Authority (MEMSA) to formally authorize the procurement of Paramedic personnel services, advanced life support equipment, and emergency medical supplies for deployment across the Philippines Manila metropolitan area. The objective of this Purchase Order is to ensure that the city of Philippines Manila maintains a fully operational, well-equipped, and professionally staffed Paramedic emergency response network capable of delivering rapid, high-quality pre-hospital care to residents and visitors alike.

The Paramedic services procured under this Purchase Order shall comply with all standards set by the Philippine Department of Health (DOH), the Philippine National Police (PNP) Emergency Response Division, and the local government units of the Philippines Manila region. All Paramedic personnel engaged through this Purchase Order must hold valid licensure from the Philippine Board of Nursing and possess current Advanced Cardiac Life Support (ACLS) and Pre-Hospital Trauma Life Support (PHTLS) certifications.

Delivery Location: All equipment and materials under this Purchase Order shall be delivered to the MEMSA Central Operations Facility located at 5655 Shaw Boulevard, Mandaluyong City, Philippines Manila, or to designated Paramedic station sites within the Philippines Manila metropolitan area as specified by the issuing authority.
Paramedic Personnel Deployment: All Paramedic personnel contracted under this Purchase Order shall report for duty within fourteen (14) calendar days of the date of this Purchase Order. Deployment schedules for the Philippines Manila coverage zones shall be coordinated between PAPS and MEMSA operations management.
Quality Assurance: The supplier warrants that all Paramedic equipment, vehicles, and medical supplies conform to Philippine FDA registration standards and DOH specifications. Any non-conforming items shall be replaced at the supplier's expense within five (5) business days.
Payment Schedule: Payment shall be made in three (3) installments: 40% upon execution of this Purchase Order, 40% upon confirmed delivery and inspection of all Paramedic equipment in Philippines Manila, and 20% upon successful completion of the 30-day operational trial period.
Warranty: All Paramedic ambulance units carry a minimum two (2) year comprehensive warranty. Medical devices and electronic systems carry a one (1) year manufacturer's warranty. PPE and consumables are non-returnable once opened.
Compliance: The supplier shall comply with all applicable laws of the Republic of the Philippines, including the Data Privacy Act of 2012 (RA 10173), the Occupational Safety and Health Standards, and local ordinances of the Philippines Manila local government units.
Termination: Either party may terminate this Purchase Order with thirty (30) days written notice. In the event of termination, the supplier shall ensure a minimum of sixty (60) days of continued Paramedic service coverage in Philippines Manila to prevent disruption of emergency medical response.
Dispute Resolution: Any disputes arising from this Purchase Order shall be resolved through mediation under the rules of the Philippine Dispute Resolution Center, with venue in the Philippines Manila region.

This Purchase Order is valid upon the signatures of the authorized representatives of both parties. By signing below, the supplier acknowledges receipt of this Purchase Order and agrees to fulfill all Paramedic service and equipment obligations as specified herein for the Philippines Manila operational area.
