Terms and Conditions of this Purchase Order
- This Purchase Order is issued in accordance with the procurement policies of Sydney Metropolitan Health Alliance and all applicable legislation under the Pharmacy Board of Australia and the Therapeutic Goods Act 1989 (Cth).
- The Pharmacist named in this Purchase Order must maintain a current, unrestricted registration with the Australian Health Practitioner Regulation Agency (AHPRA) for the entire duration of the engagement in Australia Sydney.
- All Pharmacist services shall be delivered in compliance with the Australian Commission on Safety and Quality in Health Care (ACSQHC) National Safety and Quality Health Service (NSQHS) Standards.
- The Pharmacist shall carry professional indemnity insurance with a minimum cover of AUD $10,000,000 and provide a certificate of insurance prior to the commencement of services.
- Payment shall be made within thirty (30) calendar days of receipt of a valid tax invoice. All amounts are quoted in Australian Dollars (AUD) and are inclusive of applicable Goods and Services Tax (GST) as per Australian Taxation Office regulations.
- The Pharmacist shall comply with all privacy obligations under the Privacy Act 1988 (Cth) and the Australian Privacy Principles when handling patient information within the Australia Sydney facility.
- This Purchase Order may be terminated by either party with thirty (30) days written notice. In the event of termination, the Purchaser shall be liable for services rendered up to the date of termination.
- All disputes arising from this Purchase Order shall be resolved in accordance with the laws of the State of New South Wales, Australia, and shall be subject to the exclusive jurisdiction of the courts of Australia Sydney.
- The Pharmacist shall submit monthly service reports detailing all clinical interventions, medication reviews conducted, and patient outcomes to the Clinical Pharmacy & Medication Management Department.
- This Purchase Order does not constitute an employment relationship. The Pharmacist is engaged as an independent contractor and is responsible for all applicable tax obligations under Australian law.
