This Purchase Order is issued by Ceylon Health & Wellness Group (Pvt) Ltd. to formally engage the services of a qualified Pharmacist for a period of twelve (12) months, commencing 1 August 2025. The Pharmacist shall be stationed at the primary dispensing facility located in Sri Lanka Colombo and shall be responsible for the procurement, verification, dispensing, and patient counselling of all pharmaceutical products handled by the organisation. This Purchase Order governs all terms, conditions, compensation structures, and professional obligations associated with the engagement of the Pharmacist in Sri Lanka Colombo.

4.1 — This Purchase Order is subject to the Pharmacy Act No. 27 of 1960 (as amended) and all applicable regulations issued by the Pharmacy Council of Sri Lanka. The Pharmacist must maintain an active and valid registration with the Pharmacy Council throughout the duration of this Purchase Order.
4.2 — The Pharmacist shall report for duty at the designated facility in Sri Lanka Colombo between the hours of 08:00 and 17:00 on weekdays, with additional on-call availability as specified in Line Item 02 of this Purchase Order.
4.3 — Payment under this Purchase Order shall be made on a monthly basis via bank transfer to the Pharmacist's designated account within Sri Lanka Colombo, no later than the fifth (5th) business day of the following month.
4.4 — The Pharmacist is required to comply with all data protection and patient confidentiality obligations as stipulated under the Personal Data Protection Act of Sri Lanka. All patient records handled in Sri Lanka Colombo shall be maintained in strict accordance with institutional policy.
4.5 — Either party may terminate this Purchase Order with thirty (30) days' written notice. In the event of termination, all outstanding compensation owed to the Pharmacist up to the termination date shall be settled within fourteen (14) days.
4.6 — The Pharmacist shall not engage in any concurrent employment or consultancy that conflicts with the duties outlined in this Purchase Order without prior written consent from the Buyer.
4.7 — All disputes arising from this Purchase Order shall be resolved through mediation in Sri Lanka Colombo before any escalation to the courts of law in Sri Lanka.
By signing below, both parties acknowledge that this Purchase Order constitutes a binding agreement for the engagement of the Pharmacist in Sri Lanka Colombo. The terms set forth herein shall remain in full force and effect for the duration specified, unless amended in writing by mutual consent of both parties.
