This Purchase Order is issued to formally authorize the procurement of professional Physiotherapist services for the Central Medical Rehabilitation Authority operating in Afghanistan Kabul. The contracted Physiotherapist shall provide comprehensive rehabilitation, therapeutic exercise, manual therapy, and patient education services to individuals recovering from orthopedic injuries, post-surgical conditions, neurological impairments, and chronic musculoskeletal disorders. The scope of this Purchase Order encompasses both in-clinic sessions and supervised home-visit programs within the Afghanistan Kabul metropolitan area.

  1. Service Commencement: The contracted Physiotherapist personnel shall report for duty no later than 1 July 2025 at the designated facility in Afghanistan Kabul. A minimum of two (2) weeks of onboarding and orientation shall be provided prior to independent patient care.
  2. Qualifications: All Physiotherapist staff assigned under this Purchase Order must hold a recognized Bachelor's or Master's degree in Physiotherapy, a valid practicing license issued by the Ministry of Public Health of Afghanistan, and a minimum of three (3) years of clinical experience. The Senior Physiotherapist must additionally demonstrate at least seven (7) years of post-qualification experience.
  3. Working Hours: Standard clinical hours shall be Monday through Friday, 08:00 to 17:00, with Saturday morning sessions (08:00 to 13:00) for group therapy. Overtime beyond 48 hours per week requires prior written approval from the Buyer's Director of Clinical Services.
  4. Performance Standards: The Physiotherapist shall maintain patient satisfaction scores above 85%, complete all treatment plans within the prescribed timelines, and submit monthly progress reports to the Buyer. Failure to meet these standards for two consecutive months shall constitute grounds for contract review.
  5. Confidentiality: All patient records, treatment data, and operational information pertaining to services delivered in Afghanistan Kabul shall be treated as strictly confidential in accordance with Afghan medical privacy regulations and international data protection standards.
  6. Insurance and Liability: The Supplier shall maintain comprehensive professional liability insurance covering all Physiotherapist personnel at a minimum coverage of USD 500,000 per incident. Proof of insurance shall be provided prior to the commencement of services.
  7. Termination: Either party may terminate this Purchase Order with thirty (30) days' written notice. In cases of material breach, termination may be effected with seven (7) days' notice. Upon termination, all patient records and proprietary materials shall be returned to the Buyer within five (5) business days.
  8. Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Islamic Republic of Afghanistan. Any disputes arising from this Purchase Order shall be resolved through mediation in Afghanistan Kabul before proceeding to arbitration.

All services under this Purchase Order shall be rendered at the Central Medical Rehabilitation Authority facility located in Afghanistan Kabul, Wazir Akbar Khan Road, unless otherwise specified for home-visit programs. The Supplier is responsible for ensuring that all Physiotherapist personnel have valid travel permits and security clearances required to operate within Afghanistan Kabul. Any changes to the primary service location within the Afghanistan Kabul metropolitan area must be communicated in writing at least fourteen (14) days in advance.

Important Note: This Purchase Order is subject to final approval by the Procurement Committee of the Central Medical Rehabilitation Authority. No services shall commence until all contractual documents, insurance certificates, and professional licenses have been verified and filed. The Supplier acknowledges that operating in Afghanistan Kabul may require compliance with additional local regulatory requirements, and the Supplier assumes full responsibility for maintaining all necessary permits and registrations throughout the contract period.

This Purchase Order is hereby authorized and approved by the undersigned representatives of both parties. By signing below, each party acknowledges and agrees to all terms, conditions, and obligations set forth in this Purchase Order for the provision of Physiotherapist services in Afghanistan Kabul.
