This Purchase Order is issued by Dhaka Metropolitan Health & Rehabilitation Centre to formally procure the professional services of a qualified Physiotherapist for a continuous engagement period within the Bangladesh Dhaka metropolitan area. - This Purchase Order constitutes a binding agreement between the purchasing entity and the Physiotherapist service provider. Acceptance of this Purchase Order by the Physiotherapist shall be confirmed in writing within five (5) business days of receipt.
- All services described in this Purchase Order must be rendered exclusively within the Bangladesh Dhaka metropolitan area, including but not limited to the districts of Dhaka North, Dhaka South, and Dhaka East City Corporation jurisdictions.
- The Physiotherapist shall maintain valid professional registration with the Bangladesh Physiotherapy Association throughout the entire contract period and shall provide proof of registration upon request.
- Payment shall be made in monthly installments via bank transfer to the account designated by the Physiotherapist, within fifteen (15) calendar days of receipt of a valid invoice and completion of the corresponding service period.
- The Physiotherapist is required to maintain professional liability insurance with a minimum coverage of BDT 5,000,000, valid throughout the engagement period in Bangladesh Dhaka.
- Any modification, amendment, or cancellation of this Purchase Order must be executed in writing and signed by both parties. Verbal agreements shall hold no legal standing.
- The Physiotherapist shall comply with all applicable labor laws, health and safety regulations, and professional ethical codes as prescribed by the Government of Bangladesh and the relevant regulatory authorities in Bangladesh Dhaka.
- Confidentiality: All patient records, clinical data, and proprietary information accessed by the Physiotherapist during the course of this Purchase Order shall be treated as strictly confidential and shall not be disclosed to any third party without written consent.
- Dispute resolution: Any disputes arising from this Purchase Order shall first be attempted to be resolved through amicable negotiation. If unresolved within thirty (30) days, the matter shall be referred to arbitration under the Bangladesh Arbitration Act, 2001, with the seat of arbitration in Bangladesh Dhaka.
- This Purchase Order is governed by and shall be construed in accordance with the laws of the People's Republic of Bangladesh.
The Physiotherapist shall commence all services no later than 01 August 2025 at the designated facility located in Banani, Bangladesh Dhaka. A site orientation and equipment familiarization session shall be conducted on the first day of engagement. The Physiotherapist is expected to be available for a minimum of 48 hours per week, with flexibility for emergency patient consultations as directed by the Chief Medical Officer.
By signing below, both parties acknowledge that they have read, understood, and agree to all terms and conditions specified in this Purchase Order for the procurement of Physiotherapist services in Bangladesh Dhaka.
