All services rendered under this Purchase Order must be conducted within the Canada Vancouver metropolitan area unless otherwise approved in writing by the Buyer.

1. This Purchase Order is governed by the laws of the Province of British Columbia and the federal laws of Canada Vancouver jurisdiction. Any disputes arising from this Purchase Order shall be resolved through mediation in Canada Vancouver before proceeding to litigation.
2. The Physiotherapist named in this Purchase Order must maintain valid professional liability insurance with a minimum coverage of $5,000,000 CAD throughout the duration of the engagement. Proof of insurance shall be provided prior to the commencement of services.
3. All services described in this Purchase Order must be performed by the named Physiotherapist or a credentialed associate under the direct supervision of the Physiotherapist. Substitution of personnel requires prior written consent from the Buyer.
4. Payment shall be rendered within thirty (30) calendar days of receipt of a valid invoice referencing this Purchase Order number. Invoices must be submitted electronically to [email protected].
5. The Physiotherapist shall comply with all applicable privacy regulations, including the Personal Information Protection Act (PIPA) of British Columbia and the Canada Health Information Protection standards. Patient records generated under this Purchase Order shall be stored securely in Canada Vancouver and shall not be transferred outside the province without explicit written authorization.
6. This Purchase Order is valid for a service period of six (6) months from the Required Service Start date. Extension beyond this period requires a written amendment signed by both parties.
7. The Buyer reserves the right to audit service delivery records, session logs, and billing documentation related to this Purchase Order upon fourteen (14) days written notice.
8. Early termination of this Purchase Order may be initiated by either party with thirty (30) days written notice. In the event of early termination, the Buyer shall be invoiced only for services actually rendered up to the termination date.
