- This Purchase Order is issued by Andina Corporate Wellness S.A.S. for the procurement of professional Physiotherapist services to be rendered exclusively within the city of Colombia Bógotá, specifically at the company's headquarters located in the Chapinero district.
- The contracted Physiotherapist must hold a valid professional license issued by the Colombian Ministry of Health and Social Welfare (Ministerio de Salud y Protección Social) and must be registered with the Colombian Physiotherapy Association (Asociación Colombiana de Fisioterapia).
- All services described in this Purchase Order shall be performed in compliance with Colombian labor law, specifically Decreto 1072 de 2015 and Ley 1562 de 2012, which govern occupational health and safety in the workplace within Colombia Bógotá.
- The Physiotherapist shall maintain professional liability insurance with a minimum coverage of COP 500,000,000, valid throughout the duration of this Purchase Order.

- All clinical records, patient data, and treatment documentation generated by the Physiotherapist shall remain the property of Andina Corporate Wellness S.A.S. and must be stored in accordance with Colombian data protection law (Ley 1581 de 2012).
- The Physiotherapist agrees to a non-disclosure agreement covering all proprietary information of the company and shall not disclose employee health information to any third party without written consent.
- Any modifications to the scope of services outlined in this Purchase Order must be documented in a formal amendment signed by both parties. Verbal agreements regarding the Physiotherapist engagement in Colombia Bógotá shall not be binding.
- This Purchase Order is governed by the civil and commercial laws of the Republic of Colombia. Any disputes arising from this agreement shall be resolved through the competent courts of Colombia Bógotá, D.C.
