Scope of Work and Service Requirements

This Purchase Order is issued by Golden Lotus Rehabilitation & Wellness Centre to procure the services of qualified Physiotherapist professionals and associated equipment for our primary clinic located in Myanmar Yangon. The selected Physiotherapist personnel shall be responsible for delivering evidence-based rehabilitation programmes to patients suffering from musculoskeletal injuries, post-surgical recovery, neurological conditions, and sports-related ailments. All clinical services must be rendered in accordance with the standards set by the Myanmar Physiotherapy Association and the Ministry of Health and Sports of the Republic of the Union of Myanmar.

The Physiotherapist staff engaged under this Purchase Order must hold a minimum of a Bachelor of Science in Physiotherapy (BScPT) from an accredited institution recognised in Myanmar Yangon or internationally. Senior positions require a minimum of five (5) years of post-qualification clinical experience. All personnel must possess a valid practising licence issued by the relevant Myanmar regulatory authority and must demonstrate proficiency in both Burmese and English to effectively communicate with the diverse patient population served in Myanmar Yangon.

Terms and Conditions
  1. This Purchase Order is valid for acceptance within fourteen (14) calendar days from the date of issue. Failure to confirm acceptance within this period shall render this Purchase Order null and void.
  2. All Physiotherapist services and equipment deliveries must be completed at the designated Myanmar Yangon facility address stated in this document no later than 01 August 2025.
  3. Payment shall be processed via bank transfer to the vendor's registered account in Myanmar Yangon within thirty (30) days of receipt of a valid tax invoice and confirmation of satisfactory service delivery.
  4. The vendor shall provide a minimum of ninety (90) days' written notice prior to the termination of any Physiotherapist engagement. During this notice period, the vendor must ensure continuity of patient care at the Myanmar Yangon clinic.
  5. All equipment supplied under this Purchase Order shall carry a minimum manufacturer's warranty of twenty-four (24) months. Warranty claims and technical support must be handled locally within Myanmar Yangon to minimise downtime.
  6. The vendor is responsible for all statutory employer contributions, social security payments, and occupational health and safety compliance for the Physiotherapist personnel as mandated by Myanmar labour law.
  7. Any disputes arising from this Purchase Order shall be resolved through mediation in Myanmar Yangon before escalation to the Yangon District Court. The governing law shall be the laws of the Republic of the Union of Myanmar.
  8. The vendor shall maintain professional indemnity insurance with a minimum coverage of MMK 50,000,000 for all Physiotherapist staff operating at the Myanmar Yangon facility.
  9. Confidentiality of all patient records and clinical data handled by the Physiotherapist team is strictly required in compliance with Myanmar data protection regulations.
