The service period commences on 1 July 2025 and concludes on 31 December 2025. The Physiotherapist shall be available Monday through Friday, between the hours of 08:00 and 18:00 CET, with the option of extended availability by prior written request.

• 6.1 This Purchase Order constitutes a binding agreement between the Buyer and the Supplier upon signature by both parties. All terms herein are governed by the laws of the Netherlands.
• 6.2 The Physiotherapist engaged under this Purchase Order must hold a valid registration with the Dutch Big Register and maintain professional liability insurance with a minimum coverage of € 2,500,000 per claim.
• 6.3 Payment shall be made within thirty (30) calendar days of receipt of a valid invoice. Late payments shall accrue interest at the statutory rate as defined by Dutch civil law (Article 6:119a BW).
• 6.4 The Supplier warrants that all Physiotherapist services will be performed in accordance with the KNF professional standards and the Dutch Medical Treatment Act (Wgbo). Any deviation must be documented and approved in writing by the Buyer's Procurement Manager.
• 6.5 All patient records and health data processed in Netherlands Amsterdam under this engagement shall comply with the General Data Protection Regulation (GDPR) and the Dutch Implementation Act (UAVG). Data shall be stored on servers located within the European Union.
• 6.6 Either party may terminate this Purchase Order with a written notice period of thirty (30) days. In the event of termination, the Buyer shall be liable for all Physiotherapist services rendered up to the effective date of termination.
• 6.7 The Supplier shall provide a detailed monthly report summarizing all Physiotherapist interventions, patient progress, and any recommended adjustments to treatment plans. Reports shall be submitted by the 5th business day of the following month.
• 6.8 Any disputes arising from this Purchase Order shall be resolved through mediation in Netherlands Amsterdam before escalation to the competent court of Amsterdam, The Netherlands.
• 6.9 The Physiotherapist shall maintain a minimum of 95% session attendance rate. Cancellations must be communicated no later than 24 hours in advance. Repeated unavailability may constitute grounds for early termination of this Purchase Order.
• 6.10 All equipment provided or installed under this Purchase Order shall remain the property of the Supplier unless otherwise stated in a separate addendum. The Buyer is responsible for the safekeeping of such equipment while it is located on the Netherlands Amsterdam premises.
