This Purchase Order is issued by Doha Wellness & Rehabilitation Center to formally request and authorize the engagement of qualified Physiotherapist professionals to deliver comprehensive rehabilitation and therapeutic services at our facility located in Qatar Doha. The scope of this Purchase Order encompasses the recruitment, deployment, and ongoing management of a team of certified Physiotherapists who will provide inpatient and outpatient physiotherapy services to patients recovering from orthopedic injuries, post-surgical rehabilitation, sports injuries, neurological conditions, and chronic pain management.

The Physiotherapist team engaged under this Purchase Order must hold valid professional licenses issued by the Qatar Ministry of Public Health and must possess a minimum of five (5) years of clinical experience in their respective specializations. All Physiotherapists assigned to this contract must be fluent in English and Arabic to ensure effective communication with patients and referring physicians within the Qatar Doha healthcare network.

Contract Duration: Twelve (12) months from the date of acceptance, subject to annual review. Total Contract Value: QAR 1,908,000.00 (One Million Nine Hundred and Eight Thousand Qatari Riyals).

1. This Purchase Order is governed by the laws of the State of Qatar. Any disputes arising from the interpretation or execution of this Purchase Order shall be resolved through arbitration in Qatar Doha in accordance with the Qatar Arbitration Law.
2. The Supplier shall ensure that all Physiotherapists engaged under this Purchase Order maintain valid professional registration with the Qatar Ministry of Public Health and carry comprehensive professional indemnity insurance with a minimum coverage of QAR 2,000,000 per claim.
3. Payment shall be processed within thirty (30) calendar days from the date of receipt of a valid invoice. Invoices must reference this Purchase Order number (PO-QD-2025-04782) and be submitted to the accounts payable department at Doha Wellness & Rehabilitation Center, Qatar Doha.
4. The Supplier is responsible for the recruitment, visa sponsorship, work permit processing, and annual medical fitness certification of all Physiotherapists assigned to this contract in Qatar Doha.
5. Each Physiotherapist shall adhere to the clinical protocols, patient confidentiality policies, and health & safety standards established by the Buyer. Failure to comply may result in immediate termination of the individual's assignment without penalty to the Buyer.
6. The Buyer reserves the right to request the replacement of any Physiotherapist who fails to meet the performance benchmarks outlined in the Service Level Agreement (SLA) attached as Annex A to this Purchase Order.
7. All services rendered under this Purchase Order shall be performed at the Buyer's facility in Al Sadd District, Qatar Doha, unless otherwise agreed in writing by both parties.
8. This Purchase Order may be amended only by mutual written consent of both parties. No verbal modifications shall be considered valid or binding.
9. The Supplier shall submit a monthly performance report detailing patient throughput, treatment outcomes, and any incidents or near-misses involving the Physiotherapist team to the Buyer's Quality Assurance department.
10. Early termination of this Purchase Order by either party requires a written notice of no less than sixty (60) days. In the event of early termination, the Buyer shall be liable for all services rendered up to the termination date.
