The following terms and conditions govern this Purchase Order for the engagement of the Physiotherapist in Singapore Singapore:

1. Professional Qualification: The Physiotherapist shall maintain a valid professional registration with the Singapore Physiotherapy Council throughout the duration of this Purchase Order. Any lapse in registration shall constitute grounds for immediate termination of this Purchase Order by the Buyer.
2. Place of Service: All Physiotherapist services shall be rendered at the designated facilities located in Singapore Singapore, specifically at the Meridian Wellness Centre, 12 Marina Boulevard, Singapore Singapore. The Physiotherapist shall not relocate the place of service without prior written consent from the Buyer.
3. Confidentiality: The Physiotherapist shall adhere to strict patient confidentiality protocols as mandated by the Personal Data Protection Act (PDPA) of Singapore Singapore. All patient records and clinical notes generated during the performance of this Purchase Order shall remain the property of the Buyer.
4. Insurance: The Physiotherapist shall maintain a minimum of SGD 1,000,000 in professional indemnity insurance coverage for the entire term of this Purchase Order. A certificate of insurance shall be provided to the Buyer prior to the commencement of services in Singapore Singapore.
5. Payment Schedule: Invoices shall be submitted monthly. Payment for services rendered under this Purchase Order shall be made within thirty (30) calendar days of receipt of a valid invoice.
