2. Description of Purchase

This Purchase Order is issued by Ceylon Wellness & Rehabilitation Centre (Pvt) Ltd. to formally engage the services of a qualified Physiotherapist and associated clinical support staff for a period of twelve (12) months. The Physiotherapist shall provide comprehensive outpatient and inpatient rehabilitation services at the facility located in Sri Lanka Colombo. The scope of this Purchase Order encompasses manual therapy, electrotherapy, exercise prescription, post-surgical rehabilitation, sports injury management, and chronic pain management programmes. All services shall be delivered in accordance with the standards set by the Sri Lanka Physiotherapy Association and the Ministry of Health, Sri Lanka.

4. Terms and Conditions
  • Service Location: All Physiotherapist services under this Purchase Order shall be rendered exclusively at the Ceylon Wellness & Rehabilitation Centre premises in Sri Lanka Colombo, unless otherwise agreed in writing for home-visit or hospital-based interventions.
  • Qualification Requirements: The Lead Physiotherapist must hold a Bachelor of Science in Physiotherapy (BSc PT) from a Sri Lanka-recognized institution, be registered with the Sri Lanka Physiotherapy Association, and possess a minimum of five (5) years of post-qualification clinical experience. The Junior Physiotherapist must hold a minimum of two (2) years of experience.
  • Payment Terms: Payment shall be made on a monthly basis within thirty (30) days of receipt of a valid invoice. The first payment is due on 15 July 2025. All payments shall be made via bank transfer to the supplier's designated account in Sri Lanka Colombo.
  • Performance Standards: The Physiotherapist shall maintain a minimum patient satisfaction score of 85% as measured by quarterly internal audits. Failure to meet this standard for two consecutive quarters shall constitute grounds for termination of this Purchase Order with thirty (30) days' written notice.
  • Confidentiality: All patient records, clinical data, and proprietary treatment protocols handled by the Physiotherapist team shall remain strictly confidential in compliance with the Sri Lanka Data Protection Act No. 9 of 2022.
  • Termination: Either party may terminate this Purchase Order with sixty (60) days' written notice. In the event of termination, the supplier shall be compensated for services rendered up to the effective date of termination.
  • Governing Law: This Purchase Order shall be governed by and construed in accordance with the laws of the Democratic Socialist Republic of Sri Lanka. Any disputes shall be resolved through arbitration in Colombo, Sri Lanka Colombo, in accordance with the Arbitration Act No. 11 of 1995.
  • Force Majeure: Neither party shall be liable for failure to perform obligations under this Purchase Order due to events beyond reasonable control, including but not limited to natural disasters, government-imposed restrictions, or public health emergencies affecting Sri Lanka Colombo.
5. Delivery and Commencement

The Physiotherapist services shall commence on 01 July 2025 and shall continue for a period of twelve (12) months, concluding on 30 June 2026, unless extended by mutual written agreement. The supplier shall ensure that all Physiotherapist staff are inducted, registered with the facility's HR system, and equipped with necessary credentials no later than 28 June 2025. All therapeutic equipment listed in Line Item 4 shall be delivered and installed at the Sri Lanka Colombo facility by 30 June 2025.
